AT&T Trade In Rewards Card - Never Received

I traded my phone in July 21st and the guy helping me could not apply the credits to my new phone. He said he mailed me a accessories gift card that can be used towards my bill for $165. It has been over 2 months and I still have not received the card in the mail and it hasn't been applied to my bill. 

 

I've contacted At&T over 8 times and went into the store that the trade in took place. I finally received this email and when trying to call the number given - it was OUT OF SERVICE!!! I've never dealt with a situation like this with Sprint or Verizon. WHERE IS MY GIFT CARD?
