allowable position of NID box
About the location of a fiber optic NID box: can it be located way high up near the soffit of my house, where the cable arrives from the utility pole? What's its minimum and maximum distance from the ground?

My installer initially wanted to put it where my old AT&T POTS NID was, which was about 12" off the ground. This was inside our fence/gate. Then he decided that maybe it would be better outside of the gate and up high, closer to the soffit, which I wasn't thrilled about (would prefer to keep it out of the public space). We compromised with a location back inside the gate and about 5' off the ground.
So that exercise essentially proved that anywhere on the wall was fine.
