Fiber to Guest House
We built a guest house and would like to run Fiber out there. Do we need a service appointment for someone to come do this?

Yes, you'd have to call for service. If it's not a separate postal address they won't run service to it. So, you might want to consider another carrier for it, or if it's less than 300ft run a conduit to it and pull a CAT5E or better cable to it and run it off your router, that way since it's a guest house you would have to be making monthly payment on a separate service.
