Yes, a 010 ONT… either the last customer took it or the cleaning crew removed it…
Need to call 800.288.2020 to schedule a service appointment for failed Customer Self Install (CSI) due to missing equipment.

Did you even read the Solution to this topic posted by @my thoughts ? You need to call 1-800-288-2020 and follow the VRU prompts.
