Thank you for referencing me to Lafayette's answer.  

But the problem is the current account holder does not have access to a computer (he is locked out) and has cognitive impairments.

Do you have any other suggestions because of these isuues?

Not really, that was AT&T's official stand on it. If you have access to an AT&T corporate store you could try taking all your documentation to them, it must be a corporate store, not an authorized dealer even though they display the AT&T logo.
