Phone bill

If i have a app that gives me a 2nd phone number. (I mainly use SMS to message). Will it show up on the phone bill. I downloaded a app that gave me a 2nd phone number. Now im wondering if my conversations on that 2nd phone number or app will show up on the phone bill.

If the phone number is owned by Google Voice, or some other "VOIP-type" service, then your wireless carrier has no idea about it directly.  Unless whatever "app" you've got giving you this second number is set up to bill the carrier for use of the app, then it won't show up on your bill at all.  The conversations will certainly not show up on your wireless bill.  It will be up to the app if you have access to any sort of log of such conversations.

AT&T respects your privacy

It's not so much an issue of whether or not AT&T "respects your privacy" as it is AT&T has no idea what your App is doing... it's just carrying TLS encrypted data from your phone to the App's servers, and as such AT&T has no access to the call information.
