Landlines calling plan

I've had a landlines plan called " All Distance" by AT&T California Residence Flat Rate for more than 20 years. This plan let me call anywhere in California for one flat rate.  I normally travel for work 11 months out of the year and did not notice problems. I now notice I received charges where I make a call or someone calls me.  In addition to the "All Distance flat rate" I'm charged by the minute.  I was told "All Distance" don't mean all. If been trying to work with AT&T for the past year to get this resolved.  I believe AT&T and AT&T is so large they can't keep up with all of their calling plans and this has lead to not honoring their contractual agreement with me. I'm finding it impossible to speak to any one in authority. I'm being over bill on average $100 every month.
