How do I get a phone off my account when it was 2 phone upgrades ago
I am trying to unlock an old iPhone of mine. it has been long paid off and my wife used it for years after it was paid after me, now its saying its tied to an account. how do I remove it from my account when it isn't showing in my account?

I always tell people to unlock their phone once it's paid off, and before they replace it on their service plan.
Not always a viable option.
If you're trying to unlock it as a non-customer and it thinks it's still a customer phone, try filling out the form with the yes bubble, and use the phone number that it was previously tied to. If you have tried both methods with no success, both as a customer and a non-customer then you'll have to contact AT&T directly. You can try chat
They will want the IMEI of the phone so that they can verify that it was previously on your account and is eligible for unlocking
