Please edit your post to remove your personal information. Besides, this forum isn’t how anything gets done. AT&T isn’t going to read it and do anything.

AT&T should not read it and do anything with it. When porting, you go to the NEW provider that you're transferring TO and give them the information in the venue they require, not the outgoing one.
The old provider might disconnect your phone number making it impossible to port.
All you have done (other than get us to provide some educational material) is apparently made it possible for someone else to go to the provider of their choice and port your phone number and get a SIM and own that number (since it looks like you gave them everything they need to pretend to be you to do so). Next they can use your number to steal your bank accounts, etc...
