AT&T Retireree Concession Service
Last month the submission, by mail, to Health Smart, P.O. Box 11296, Charleston, WV 25339-1296 for toll reimbursement was returned as "not deliverable as addressed".
Has there been an address change? Please advise.

That's the address I have and my haven't had any returned. Only difference is in the name, I show HealthSmart Benefit Solutions, but that difference shouldn't cause it to be returned.

Thank you. I used "Health Smart Benefit Solutions" with the same address and so far my mail has not been returned.

My April bill was returned as undeliverable, I called the benefits center at 877-722-0020 and was given a new address as TOLL Reimbursement Center, PO Box 99010, Lubbock, TX 79490
I hope this is right because I'm going to mail my bill there, If anyone has anything different please reply.

I mailed my April bill to the Charleston, WV address and it was returned as undeliverable, so I called the benefits center at 877-722-0020 an was given an address of Toll Reimbursement Center, PO Box 99010, Lubbock, TX 79490. I hope this is correct because I'm going to send my bill there. If anyone has anything different please reply.

My tool reimbursement last 2 mailings just disappeared into thin air & my return address was definitely on the envelopes.

I still use: HealthSmart Benefit Solutions, Inc.
PO Box 99010
Lubbock, TX 79490
and haven't had a problem to-date. Note: the complete addressee is important as HealthSmart provides other services.
