Landline Missed calls

We have AT&T Office at Hand system.  Many times a day I get an email notification that we had a missed call.  However, these can come when I'm sitting at my desk in front of the phone and I know very well that it did not ring.  What can be the issue and how can it be resolved?  We are losing business due to the frequency.
