Conditional Call Forwarding not working on iPhone

I have used conditional call forwarding for years 61, 62, and 67. With the latest iOS update this feature does not work. Unanswered calls for example still get voicemail left on my iPhone and don’t get forwarded to my other phone number. *21 (forward ALL calls) works, but this doesn’t suit my needs

I am trying to enable Wi-Fi calling on my device. I get an error "Need Valid Address" with the code GEO-0074-WS The address I entered matches EXACTLY what is on the Zip Code lookup from the USPS. How can this be resolved?
